New TXA Consensus Statement

Recently, a joint position statement on the use of tranexamic acid (TXA) in prehospital trauma care was collaboratively created by the National Association of EMS Physicians (NAEMSP), the American College of Surgeons Committee on Trauma (ACS-COT), and the American College of Emergency Physicians (ACEP). This document updates previous guidance and recognizes the significant rise in prehospital TXA use despite conflicting evidence about its overall effectiveness.

Based on a review of the literature, the three organizations offer several evidence-based recommendations, primarily suggesting that prehospital TXA administration may decrease mortality in adult trauma patients with hemorrhagic shock when given within three hours of injury. The article also discusses the safety profile, optimal dosing, and patient selection criteria, while highlighting the uncertain role of TXA in pediatric trauma patients and emphasizing the need for comprehensive quality improvement programs.

Is there a role for ECMO in trauma patients?

Extracorporeal membrane oxygenation (ECMO) is a life-sustaining therapy that has found clinical use in severe heart and lung disease cases. Many critically ill COVID-19 patients were successfully treated with this therapy. But is there a role for ECMO in managing trauma patients? 

Although its use remains controversial due to limited availability, high costs, and the risk of anticoagulation complications, ECMO can provide temporary support for select trauma patients, especially those with severe lung injuries, ARDS, or significant trauma-related cardiopulmonary failure (e.g., massive pulmonary embolism).
